This central church of the town of Lastovo was built in the 14th century, on the site of another church dating back to 5th or 6th century.

It started off as a single nave building, with the central nave built in the 15th century, and then expanded during the 16th and the 17th centuries, when the other two naves were added, respectively. Each of the naves has its own bell fry, built in the 18th century and possessing the characteristics of the Gothic style. The interior of the church is rich with decor and artefacts from all the stylistic periods. This diversity is best seen in the chalices, stored in the vault, as well as candle holders and various crosses.

Of particular value are the Gothic-Renaissance chalice and the container for Holy Water made in the Renaissance style. The main altarpiece is a painting of St. Cosmas and Damian done by Giovani Lanfranco, while the author of the impressive Pieta remains unknown.
